Getting to and from Prague

since the Eurorail doesn't cover Czech Republic as one of the countries on the list of 17 countries covered, what is the best way and cheapest way to get to Prague from a) Switzerland (Zurich) b) Italy (Venice or Milan) or c) Austria (Vienna)

and what is the best and cheapest way to get from Prague to Germany (Berlin)
