In my experience in Italy, I found on several occasions that even many younger people could not speak much (if any) English, while some older / middled aged people could speak a bit of English (or were quite fluent). Having some basic Italian language skills is a definite asset! I agree though that generally speaking one will more likely find that younger people (especially in large cities) will be able to speak some English.
With regard to the safety aspect of travelling in Naples, one other point comes to mind. I recall that sometime last year, a Canadian Diplomat was found deceased in a storm sewer in Naples. He was posted to one of the Canadian Embassies in Europe (can't remember which one?). I don't believe the newspaper articles said whether he was there on business or vacation?
To stress what the others have said about the "bare shoulders" issue in Churches, the Italians are VERY STRICT about this, especially in the Vatican! The Guards there are quite intolerant with anyone that forgets.

Hi! Ok, Naples hasn't got a good fame..but I love it, and think that everyone should have the oppotunity to see its gulf and vesuvio from the via caracciolo "lungomare"..breathtaking. Anyway, yes, it can actually be "unsafe", but it depends from where you stay. for example, avoid the "quartieri spagnoli" at night - pickpockets &C. are a real business there, but for the rest..people is sunny and friendly, anyway walk around but remember that if you've got a too touristic air you'll be an easy target (maps, cameras &C..hide them). see pompei - it's amazing - and remember that you can't say that you've been in naples if you don't eat a good Pizza - with the capital P have a nice trip!

I agree with lia completely. I think, for any major city, you just have to be conscious of your surroundings. Act like you know where you are going, walk with purpose and blend in with those around you. Learn a little Italian before you go so that you won't have to rely on other people for directions. I loved Naples. It is vibrant and hot and colorful and the people (most of them) are friendly and interesting.
Just be on alert and I think you'll have a wonderful time! And also take some of the others suggestions and think about finding another backpacker to travel with for awhile.
